Welcome to the "My Chain Delivers" survey and thank you for agreeing to participate. All your answers will be kept confidential and used only in aggregate.
The questionnaire consists of a qualitative and a quantitative part. Each part contains 9 sections. Sections 1 and 2 examine the performance of your company/business unit and your supply chain. Section 3 covers the overall supply chain strategy and organisation, while Sections 4 to 7 focus on concrete SCM practices. Section 8 then assesses your information and communication practices. In the last section, we ask some industry-specific questions and confront you with current supply chain trends.
After clicking the button below, you will be directed to the questionnaire. Then, you will be asked to enter your password. Should you not yet have received your password, or if you have any other questions related to the survey, please contact Daniel Rexhausen from the Supply Chain Management Institute.
- Please do not use the navigation buttons on the top of your internet browser. Use survey "Back" and "Next" buttons provided at the bottom of the page to navigate the survey
- In case you mistakenly press the internet browser navigation buttons, press the "Refresh" button twice to resume the survey
- If you want to leave the survey and continue later, click on the "Save" button before closing your internet browser. You can resume the survey by pressing the "Begin Survey" button again. Note: you will have to use the same computer to complete your saved survey
- You can go back and change answers at any time, until you press the "Finish" button at the end of the survey
